Using Care Coordination in Medicaid Managed Care


As states enroll increasing numbers of people with disabilities and special needs into Medicaid managed care, they are devising innovative approaches to ensure that both psychosocial and medical needs are met. This coordination reaches across different parts of the health care and social services system to help people with special health care needs, especially those with chronic conditions, navigate through the complex Medicaid managed care environment. 

A new policy brief from Mathematica, "Care Coordination and Medicaid Managed Care: Emerging Issues for States and Managed Care Organizations," by Margo Rosenbach and Cheryl Young, examines the potential of care coordination. The brief notes that care coordination takes time to develop but can be put in place even after a state has implemented Medicaid managed care. However, to be effective, it must be broader than simply expanding case management to include referrals for social services.
